Health Assessments

The Health Assessment grant is available to fund the cost of a Health Assessment with Nuffield Health who are the UK’s largest not for profit healthcare organisation with 50 assessment centres across the UK. The grant is available to the beneficiary (former employee of MBC and their spouse).  The assessment is not suitable for, or available to, dependent children.  Subsequent assessments are available every three years on the anniversary of any previous assessment.

The assessment available is the PATH Pro 4 and is the most in depth and detailed health analysis journey available from Nuffield.

The Health Assessment Journey

Health Risk Analysis – You will complete your Health Risk Analysis in a safe, secure environment online.  The output will identify your individual risk, and signpost you to further content, information and services whilst creating your own bespoke health assessment. The output will also recommend the health modules which best suit your needs based on your medical background/input. You can choose different modules if you wish at this stage.

Health Assessment Delivery – You will have a face-to-face health assessment following the recommendations provided from your completed Health Risk Analysis.

The face-to-face element of the Health Assessment is carried out at a Nuffield Medical Centre that may or may not be on the same site as a Nuffield Gym/Leisure centre.

The face-to-face assessment will consist of 90 minutes with a health and wellbeing physiologist as well as 60 minutes with a health assessment doctor.  These assessments include a full blood test, key body measurements, one physiology module of your choosing and two medical modules of your choosing. The modules are selected at the end of the online questionnaire – see above.

Application guidance

The grant is to fund a health assessment.  Any health issues or concerns flagged with you  at the assessment must be referred to your GP.  There is no further grant funding available for accessing any private consultations/appointment or tests etc. through Nuffield.  You will be responsible for any additional costs you incur.

If the application is for the spouse or partner of the former employee of Mansfield Brewery, the application form for SPOUSE should be completed. A copy of the marriage certificate, and current (within a maximum of two months) utility bill or other official documentation evidencing that the spouse/partner and former employee live together, should be sent with the completed application form. The Nuffield’s computerised Health Risk Analysis is completed online, and each applicant must have their own personal email address.

Your application will not be processed if you do not provide all the information requested. We will contact you if we require any further or additional information.

You will receive an acknowledgment of receipt of your application normally within one week of receipt.

Grant decision

Applications are sent to the trustees on a weekly basis and a response is usually provided within approximately two/three weeks although you should be aware that at times it may take longer.

Confirmation, in writing, of the outcome of your application, will be sent as soon as a decision is given by the Trustees.  If a grant is awarded, details of how to access the assessment will be included in the grant offer letter.  

Grants are discretionary

You should note that all grants are discretionary, and the trustees review their policies regarding the allocation of benefits regularly.  This means that grants can be added and withdrawn at any time by the trustees.  As trustees of a discretionary settlement, the trustees cannot give a commitment to provide benefits indefinitely.
