What the service includes
This service includes:
- A personal visit from an experienced care adviser
- A comprehensive care or nursing assessment, carried out in a comfortable and familiar setting

A detailed written report outlining:
- Your current care needs
- Whether your existing arrangements are meeting those needs
The report may also include recommendations for changes or additional support.
It may also consider your future care needs.
Where appropriate, the service can include ongoing monitoring and review.
How to access the service
To access the Care Assessment and Monitoring Service, you must first complete a Care Guidance and Advice consultation with Grace Consulting.
Following this, Grace Consulting will confirm whether the service would be beneficial based on your needs and circumstances.

If the service is recommended, you will need to:
- Complete an application form in full
- Submit your application to us

You can also download the application form below and return a hard copy to the BTPF office, or request a hard-copy form from the office team.
Once approved, you will receive details of how to access the service and book your initial consultation.
All grants are awarded at the discretion of the trustees, who will consider each application based on the information provided.
