How the service can help
The Care Guidance and Advice Service is funded by the Baily Thomas Provident Fund and delivered by Grace Consulting, who have many years of experience supporting people with care-related decisions.
You can speak with a specialist adviser about a wide range of care-related matters, including:
Through a personalised care consultation, beneficiaries can speak with a specialist adviser who can offer support and guidance on a range of matters, including:
- Understanding the different care options available
- Support following discharge from hospital, and help navigating the care system, the NHS, and social services
- Identifying the most suitable care providers
- Paying for care, including fair costs of care and state funding
- Maintaining independence at home and helping to guard against potential future crises
- Home adaptations and assistive devices
- Guidance and support for carers
- Dementia care and support
- Legal aspects of care, including your rights and the responsibilities of the state
Your dedicated care adviser can:
- Listen carefully to your needs and wishes
- Discuss your situation in detail
- Explain the care options available to you
- Research suitable care providers, where appropriate
- Make you aware of statutory services and funding for which you may be eligible
- Guide and support you through the next steps
Support is available whether you are a carer or a beneficiary who needs care yourself.

How to access the service
To access the service, complete a short opt-in application form for trustee approval.
The easiest way to do this is via our online grant application portal: https://benefactorcloud.co.uk
You can also download the application form below and return a hard copy to the BTPF office, or request a hard-copy form from the office team.
Once approved, you will receive details of how to access the service and can self-refer whenever you need support.
If additional services are recommended, a further grant application form will need to be completed.
